POSITION SUMMARY:
This position serves as the clerical support to the intake process for the Visiting Nurse and/or Hospice home care and hospice locations. This position assists the Skilled Nurse Intake Specialist in ensuring the effective communication between Home Care Coordinators, intake staff, referral sources, physicians and the SCAH organization. Referral source interactions include, but are not limited to, working closely with the SCAH Home Care Coordinators, physicians, board and care facilities, other referral sources, patients, SCAH management and other staff and the general public. The Intake Specialist receives general referrals from external sources for nursing & therapy services, prescreens new referrals and initiates the information gathering process into the patient information database, Horizon, the information into the billing system, as well as initiating and maintaining new files and associated duties.

MINIMUM POSITION REQUIREMENTS:

Education:
High school diploma or equivalent with successful completed course work or equivalent experience in medical terminology &/or working in healthcare environments.

Experience:
Minimum of two years’ experience working in a health care environment required. Customer service background, preferably via telephonic communications extremely helpful. Proven customer relations and phone etiquette abilities required. Use of computer systems, office machines and a working knowledge of phone and filing system essential.

Knowledge:
Knowledge of basic Medical Terminology required. Able to operate copy machines, basic computer programs, fax machines, word processing and e-mail. Excellent communication skills, written and verbal required. Ability to professionally handle confidential information, comply with HIPPA regulations and maintain workflow systems required.

Special Skills/Equipment:
Strong data entry and general computer skills required. This position maintains a 5% or less data entry error. Must be able to communicate effectively, to perform multiple tasks effectively and efficiently, to work with minimal supervision, to react to change productively and to handle other essential tasks as assigned. Must be able to handle sensitive issues, conflict with or among others, respectively direct and reinforce staff efforts; effectively plan and organize and prioritize work, and to effectively solve unique problems as they arise or identify when to consult supervisor. Must be able to deal with a challenging work environment from time demands and conflicting priorities. Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.
