Open Enrollment Period: March 28 2021 - April 27 2021

We are now offering Essential StaffCARE! Your annual Open Enrollment opportunity to add, drop, or change any of your benefit plan options starts March 28 2021.

If you are already enrolled in the Fixed Indemnity Medical Plan or the MEC Wellness/Preventive Plan and would like to make any changes such as adding dependent(s) etc, please use the change forms below:

Please read the plan carefully. Certified is not liable for any insurance plan decision that you make.

Because you are charged a month in advance before your coverage takes effect, you will see the updated MEC rates in your monthly deductions.

This Fixed Indemnity Medical Plan does not qualify as minimum essential coverage as defined under the Affordable Care Act (ACA). This plan is a supplement to health insurance and is not a substitute for major medical coverage. Lack of major medical coverage (or other minimum essential coverage) may result in an additional payment with your taxes.

The MEC Wellness/Preventive Plan is a self-funded plan that has been deemed to be in compliance with ACA rules and regulations.
